Sports Physicals and ImPACT Baseline Testing Info
Sports Physicals and ImPACT baseline testing are done on an annual basis. Until completed along with registration on Family ID, athletes will not be eligible to participate in CMS or SHS sports. New athletes and incoming freshmen are expected to bring a copy of their most current physical. A 14 day "grace period" will be granted to those who are unable to schedule an appointment before their current physicals expiration date. A note from your athlete's PCP confirming an appointment for a physical within 14 days is required to be qualified.
If an athlete does not have a valid physical, they will be considered ineligible to participate until a new one is completed and handed in to their designated coach or the Athletic Trainer. This is to ensure athlete safety.

ImPACT tests are completed prior to the start of each season for all sports as scheduled by the Athletic Trainer. Once an athlete has taken their baseline at the start of their first season of the school year, the baseline scores are valid for the rest of the year. They are not repeated at the start of every season. Look out for announcements on our home page for testing times prior to the start of each sports season!
